Many men are not satisfied with their penis size and would like a bigger or longer penis to please their partner and boost their confidence. It may be an embarrassing topic, but it is a popular topic and surgery in Thailand. Penile enhancement and penis thickening is one of the most request plastic surgery for men.
Penis enlargement, sometimes euphemistically called male enhancement, refers to an assortment of techniques intended to increase the girth, length, or erectile rigidity of the human penis.
Penis size varies and what is consider large or small is up to the person. Everyone is able to lengthen or thicken their penis, however most requests are for cosmetic reason instead of a necessity. Non-cosmetic reasons for penis surgery include to penis shrinkage due to scar tissue formation from a condition called Peyronie’s disease which affects up to 10% of men. In such medical conditions of micropenis, surgery is required to increase the organ’s size is sometimes needed for urinary or sexual purposes.
Procedures to enhance the penis range from manual exercises to stretching devices and surgical procedures. On the internet, there are many claims and hoaxes that men need to be aware of. We have discussed some of these topics below.
The following techniques are available to lengthen the penis.
Phalloplasty Penis Enlargement
Penile lengthening techniques or phalloplasty have been described in the urological literature for at least 40 years. Average erect lengths measured with a ruler placed gently on the pubic skin vary from 5 3/8 to 5 3/4 inches depending upon the city and year the study was performed. Many men with below average measurements may still appear normal and function quite well. For this reason, penile enhancement or phalloplasty should be considered an elective procedure.
Penis Size After Surgery
Well motivated patients who follow post-operative instructions have achieved penis length gains up to 2 inches with the average increasing penis length about 1 inch.
Men with prominent suprapubic fat pads will gain extra penis length. For example, if they have a 2 inch thick fat pad and 1 1/2 inch is liposuctionable, their total gain will be 1 1/2 inch plus whatever is gained by resecting the penile suspensory ligament.
Penis Implant Surgery
Many men with ED have limited success with medications, and many men are unsure that they will be able to have intercourse successfully on any given day. Because of the uncertainty of success, and to prevent disappointment for their partners and themselves, many men are reluctant to initiate sex. A device implanted into the penis, known as a penile prosthesis, can restore erections in most men with ED. Many men don’t realize until after penile implant surgery that they had been avoiding sexual relations because they feared that they would be unsuccessful. After implant surgery men are confident that they will be able to have intercourse successfully whenever they wish to.
There are two main types of penile implants, inflatable and malleable (also called semi-rigid). Inflatable implants consist of paired cylinders, which are surgically inserted inside the penis and can be expanded using pressurized fluid (see figures 1 and 2). Tubes connect the cylinders to a fluid reservoir and a pump, which are also surgically implanted. The patient inflates the cylinders by pressing on the small pump, located under the skin in the scrotum. Inflatable implants expand the length and width of the penis when the device is inflated. They also leave the penis in a more natural state when not inflated. MBUA urologists recommend impantation of prostheses from Mentor Corporation or American Medical Systems.
Penis Size After Implant
After penile prosthesis surgery, men are able to have intercourse with confidence that they will be able to have an adequate erection. The pump in the scrotum is pumped until the penis is hard enough for intercourse, and men keep an erection until they decide to deflate the prosthesis. There is no pain during pumping of the device.
The feeling of the erect penis may be slightly different than a natural erection because the penis is filled by the prosthesis, not blood, but it should not feel any different to the sexual partner. The feeling of climax, or orgasm, is exactly the same as it was with a natural erection. The overwhelming majority of men are extremely satisfied with the results of penile implant surgery. We have a number of patients who are willing to share their experience with men who are considering this type of treatment.
Penile implant surgery is performed as outpatient surgery; patients do not need to stay in the hospital. The surgery is performed under spinal or general anesthesia. An incision is made across the top of the scrotum at the base of the penis, and in most men the entire operation is performed through this one incision. In some men who have had previous surgery it may be necessary to make a small incision in the groin area to place the reservoir that holds the fluid.
The prosthesis is left inflated overnight after surgery and a catheter is left in the penis so that men do not need to urinate. The prosthesis is deflated in the office the next day and the catheter is removed. We recommend that men place an ice pack on the penis and scrotum for the first 2-4 hours after they go home from surgery. Men will take antibiotic tablets for 4-5 days after surgery, and will usually use pain medication for the first 2 weeks. Most men say that the pain level after this type of surgery is moderate. It is common to have some bruising and swelling of the penis and scrotum after surgery, and it will resolve like a bruise anywhere else in the body.
When the swelling and tenderness of the pump have decreased the patient will be taught how to inflate and deflate the prosthesis twice a day. Men are usually able to start to have intercourse 6-8 weeks after surgery, but it is not uncommon to have some discomfort for up to 12 weeks.
Penile girth enhancement can be performed by one of three techniques:
- Placement of dermal-fat grafts which are harvested from the lower abdomen or buttocks area
- Insertion of AlloDerm Strips
- Insertion of liposuctioned fat
- Soft silicone (irritation of the silicone may cause friction seroma and interfere with the erections)

The fat injection is the safest method but there is a problem about injecting the fat because 50-60% of the fat is absorbed in the body. Moreover, the unevenly absorbed cause the penis to be crooked and unnatural.
Penis surgery is performed in the hospital by general anesthesia and requires 1-2 days of hospitalization for postoperative care, wound dressing, intravenous antibiotics administration and monitor any unexpected complications.
Moreover, the surgeon will give a daily wound dressing. The drainage will be removed on discharge. You will require follow up with the surgeon and stitches off in 7 days after surgery.
The patients should abstain from drinking alcohol, tobacco for 2 weeks and sexual intercourse and masturbation for 6 weeks. Complications such as bleeding or friction seroma can happen but rare. Experienced surgeons found some patients improving after their erectile dysfunction (ED) after surgery becomes more increasing. Surgeon may suggest the patient to use 200 gm. penile weight for 3-4 days after surgery to maintain the penis length before using the vacuum pump 2-6 weeks later.
Penis Enhancement Package
When length and girth enhancement are requested, these procedures should be staged to get the best results from both. The use of penile traction post-op makes for appreciable length gains beyond what surgery alone can produce. A longer penis deserves a longer graft or AlloDerm product. To stretch these products from the “git-go” of lengthening is to give the penis and you an unnecessary work assignment. The simultaneous transfer of fat with lengthening will cause “thumbprints” or impressions in the newly injected fat when traction is applied. Again, these procedures must be staged.
Penis Enlargement Scams
Often, in the advertising of fraudulent products, the distinction between temporary enlargement, i.e. erection, and permanent enlargement, is deliberately muddied. Widespread private concerns related to penis size have led to a number of folklore sayings and popular culture reflections related to penis size. Penis panic is a form of mass hysteria involving the believed removal or shrinking of the penis, known as genital retraction syndrome.
Little legitimate scientific research has been done specifically on the subject of non-surgical penile enlargement; existing claims of significant and permanent enlargement are generally anecdotal, unverified by objective research, and can be biased by the interests and emotions of the source. At present, there is no proof of any non-surgical technique that permanently increases either the thickness or length of a normal penis.
